Who we are

Align & Thrive Spine Wellness is a mobile chiropractic clinic based in Gilbert, Arizona, serving 55+ communities, RV resorts, gyms and employers in Mesa, Gilbert and Queen Creek. This policy explains what information we collect through this website and our booking system, why we collect it, and the choices you have.

Information we collect

• Contact information you give us, such as your name, phone number, email address and, when you book a home or community visit, the address where you will be seen.
• Appointment details, such as the service you booked, the date and time, and any notes you add when booking.
• Health information you share with us in the course of your care, including your history, symptoms, examination findings and imaging.
• Insurance and billing information, such as your Medicare Part B details, used to bill for the care you receive.
• Basic technical information that our web server and booking system record automatically, such as the pages you visit and the type of device you use.

How we use your information

• To schedule, confirm, remind you about and provide your care.
• To answer your questions and follow up after a visit.
• To bill insurance, including Medicare Part B, and to keep the records we are required to keep.
• To send appointment messages by text or email when you have agreed to receive them.
• To keep this website and our booking system working and secure.

Health information and HIPAA

Your health information is handled under the Health Insurance Portability and Accountability Act (HIPAA). Our Notice of Privacy Practices describes how your health information may be used and disclosed, and your rights over it. Text messages

If you tick the text-message box when you book, or ask us in person, we may text you about your appointments, such as confirmations, reminders and scheduling questions. Consent to receive texts is not required to book or to receive care. Message frequency varies, and message and data rates may apply. Reply STOP to opt out or HELP for help. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.

Email communications

When you book an appointment and provide your email address, we use it to send appointment-related messages, such as confirmations, reminders, rescheduling notices, and cancellations. We use Amazon Simple Email Service (Amazon SES) to deliver these messages.

We do not sell or rent your email address or use purchased mailing lists for these appointment messages. Providing an email address for appointment communications does not enroll you in promotional email.

Please do not send sensitive health information by ordinary email. Our handling of protected health information is described separately in our Notice of Privacy Practices, available on request.

We do not sell your information

We do not sell personal information, and we do not share it for others to advertise to you.

When we share information

We share information only as needed to run the clinic and as the law allows, for example:

• With the service providers that host this website, run our booking calendar, deliver our email and text messages, and process billing, who may use it only to provide that service to us.
• With your insurance plan, including Medicare, to bill for your care.
• With other health care providers involved in your care, when you ask us to or when the law permits it.
• When we are required to by law, or to protect someone's safety.

Text messaging opt-in data and consent are excluded from any sharing described above.

How long we keep information

We keep clinical and billing records for the period Arizona and federal law require. We keep booking and contact information only as long as we need it for the purposes described here, then dispose of it securely.

Security

We use administrative, technical and physical safeguards to protect your information, including limiting access to the people who need it to do their jobs. No method of sending or storing information is perfectly secure, but we work to keep yours safe.

Your choices and rights

• You can stop appointment texts at any time by replying STOP to any message from us.
• You can ask us what information we hold about you, ask us to correct it, or ask questions about how it is used, subject to the limits in our Notice of Privacy Practices and the law.
• You can use this website and call us without giving consent to marketing or promotional messages; we do not send them.

Children

This website is meant for adults arranging care for themselves or for a family member. We do not knowingly collect information online from children.

Changes to this policy

If this policy changes, we will post the new version here and update the date at the top of the page.
